DESCRIPTION:Flourishing Together is a health education and support program 
 hosted by the Trager Institute Optimal Aging Clinic that promotes healthy 
 lifestyle choices to help you manage and prevent chronic diseases. \n\nAl
 ong with a small group of family, friends and colleagues, participants mak
 e health goals that are achievable in a fun and friendly environment. \n\
 nHow it works:\n\n\n	Team-Based Approach: Small teams of two to six family
  members, friends, or colleagues form a group. They set goals to improve t
 heir health and support each other.\n	8-Week Program: Groups meet over eig
 ht weeks to learn how to make healthy lifestyle changes. \n	Meets virtual
